Replacement of Membership Card

The replacement of a lost or misplaced Membership Card shall incur a fee of $5.00.

Where a member loses his/her membership card they must make a request (in writing) to the Registrar, SHC WA for a replacement card.

The fee of $5.00 must accompany the request, which must include the:

Member’s name;
Member’s address;
Member’s phone number; and
Member’s email address.

Please Note: A request for a replacement membership card may take up to 2 weeks to be processed.
